Microalbuminuria (MAU) is defined as urinary albumin excretion exceeding the normal reference range without meeting the diagnostic criteria for clinical proteinuria. Urinary microprotein assay represents the most sensitive and accurate diagnostic tool for screening early nephropathy, which is widely adopted clinically to monitor renal injury. Integrating urinary microalbumin test results, predisposing factors, clinical manifestations and medical history enables clinicians to clarify the severity of renal lesions and determine the staging of renal fibrosis, which is of great significance for the prevention, treatment and early intervention of kidney diseases.Albumin is primarily synthesized in the liver and composed of 585 amino acid residues, presenting an overall heart-shaped three-dimensional conformation dominated by alpha-helices. This distinctive structure endows albumin with outstanding flexibility and structural stability, so its conformation rarely alters even under extreme environmental conditions. The albumin molecule contains numerous binding sites capable of binding various endogenous and exogenous substances such as fatty acids, endogenous hormones and exogenous drugs. Accordingly, albumin not only functions as a substance transport carrier, but also participates in the regulation of diverse physiological activities. Protein function: Binds water, Ca(2+), Na(+), K(+), fatty acids, hormones, bilirubin and drugs (Probable). Its main function is the regulation of the colloidal osmotic pressure of blood (Probable). Major zinc transporter in plasma, typically binds about 80% of all plasma zinc (PubMed:19021548). Major calcium and magnesium transporter in plasma, binds approximately 45% of circulating calcium and magnesium in plasma (By similarity). Potentially has more than two calcium-binding sites and might additionally bind calcium in a non-specific manner (By similarity). The shared binding site between zinc and calcium at residue Asp-273 suggests a crosstalk between zinc and calcium transport in the blood (By similarity). The rank order of affinity is zinc > calcium > magnesium (By similarity). Binds to the bacterial siderophore enterobactin and inhibits enterobactin-mediated iron uptake of E.coli from ferric transferrin, and may thereby limit the utilization of iron and growth of enteric bacteria such as E.coli (PubMed:6234017).
